The post is NHS funded through Meir PCN on a rolling basis. It is offered as a permanent contract subject to the continuation of this funding

Role

The role of the Health & Wellbeing Coach is to provide One to One bespoke support to local residents living in Meir, wanting to overcome barriers and improve their health and wellbeing. The role will support with NHS health checks and will also involve supporting an increase in local health and wellbeing group activities either through the direct delivery of sessions or supporting local groups to create new provision. This will include delivering chair based exercise, low/medium impact sessions and befriending walks.

The role requires partnership working with the Employment & Skills Coach and the ability to work on an outreach basis within the community. This role will involve remote working and occasional visits with CRT Engage Five Towns.

To contribute towards the delivery of the CRT’s Strategy, England’s Delivery Plan and overall objectives by supporting and delivering CRT activities e.g. events, one to ones, workshops etc. that respond to the challenges that exist in former mining communities; linking local people to support available and to other organisations.
