Role Details

The Coalfields Regeneration Trust (CRT) is the only community wealth building charity dedicated to the regeneration of Britain’s coalfield communities. For over 25 years, the CRT has worked with communities to deliver a breadth of projects creating opportunities for people, strengthening the role of community organisations and identifying place based solutions responsive to the challenges still experienced in former coalmining towns and villages. To support delivery, the CRT has developed an innovative community wealth-building model generating sustainable income to deliver both social and economic value. Enabled by engaging key stakeholders, partners and, most importantly the communities, activities delivered are personalised to meet the needs of the people, the organisations and the place.

Role

The Coalfields Regeneration Trust is seeking a passionate self-starter to support delivery of the CRT Together programme to improve outcomes for people affected by cancer and the services designed to support them in the Cwm Taff Morganwg University Health Board area. If you are interested in a role which is flexible, challenging and rewarding then this may be the role for you.

In a typical day you may:

  • Complete and Offer 1-1 individual support to people living with Cancer.
  • Complete the Electronic Holistic Needs Assessment with people living with Cancer to form a comprehensive care plan.
  • Engage and scope services to enable support to be both effective and efficient given to people living with Cancer.
  • Complete and carry out action plan and be able to manage a large caseload, sign post to existing services, support in-group activities, and look to be able to break down barriers for people living with Cancer.
  • The role will require you to manage your own workload effectively, be flexible with occasional evening work and occasional weekend work. You will need to be able to work on your own initiative whilst benefitting from being part of a supportive team and renowned national charity.
  • Strong understanding of Cwm Taff Morganwg University Health Board area and its community services.
