CRT Together is our programme delivered in partnership with Macmillan Cancer Support.
The programme has been set up with the aim to support people living with cancer in Merthyr Tydfil, Rhondda Cynon Taff and the 3 Valleys of Ogmore, Llynfi and Garw, located within the Cwm Taf Morgannwg University Health Board area.
People impacted by cancer can sign up for CRT Together – either by contacting us yourself or being referred to us from another organisation.
Following a Holistic Needs Assessment, we can then create a personalised care plan, which will help link you to a number of support services within the Cwm Taf Morgannwg area, centred around your needs. Our link workers can support you to access a range of services including:
– Transport
– Financial Advice
– Family Support
– Housing Needs
Together, we can improve the cancer journey, through regular catch ups and be there for you on your journey. Whether you’d prefer to contact us through a friendly phone call, text or e-mail – it’s entirely up to you!
